Abstract

A study conducted to identify the common charting errors of DLSU-A physical therapy in their initial evaluation of cerebrovascular accident (CVA) patients at DLSUMC-DIMRU Dasmariñas, Cavite. 70 CVA patient chart’s clinical supervisors. Gathered data evaluated using a checklist and presented in tables. Percentages also computed and all data analyzed descriptively. Results showed that the highest percentage of omitted items were occupation and handedness on the demographic data while temperature, synergies, shoulder subluxation, shoulder hand syndrome, pathologic reflexes, postural analysis, skin turgor, trophic changes, secondary complications, eyes/ears/nose/throat and chest/lung assessment on physical examination. Besides, that the most significant to the physical therapy program incompletely evaluated were a range of motion, manual muscle testing, gait and activities of daily living.
