Abstract
This study employed a descriptive design using survey technique. Initial evaluations (I.E.'s) of cerebrovascular accident patients done by 14 respondents were DLSU P.T. interns from different four (4) affiliated centers of De La Salle University chosen through simple quota sampling. Evaluated according to completeness based on a Standardized Cerebrovascular Accident Assessment Form (SCAF). The survey instrument Data were gathered and interpreted using frequency, percentage, and mean. The instrument used pre-SCAF and SCAF periods and the results of both periods were compared. The researchers conclude that the standardized CVA assessment form maximized completeness of I.E.'s made by P.T. interns during charting. That another study be conducted using the revised SCAF in a longer study period with a greater study population after an initial pre-testing That a post-SCAF period be implemented with the same study population involving analysis of initial evaluations of cerebrovascular accident patients done after the user of the SCAF. This is to assess whether the physical therapy interns' initial evaluations are still complete even without the use of the SCAF. That an opinion survey be conducted among interns who used the SCAF to pinpoint specific areas of difficulty in filling up the SCAF and reasons for such difficulty to be able to discover means of facilitating accomplishment of the SCAF in the future.
