Abstract
This case of Leonicia Esguerra, a 50 year old married, female stroke patient aims to present the importance of follow-up care in post-stroke patients. It is hoped that this case report may help very member of the rehabilitation team realize that rehabilitation of stroke patients does not end in the hospital and that the improvement in function depends upon the environment in which the patient with completed stroke is placed and how much training and motivation there is for the patient to learn to become independent again in self-care, mobility and in other aspects of Activities of Daily Living (ADL). The respondent is a diagnosed case of Cerebrovascular Disease (CVD), Intracerebral Bleed, (R) Hemiplegia. During subsequent visit days by the researchers, patient and family were given suggestions and instructions on modifications of some of the patient's tools and environment to better facilitate patient's achievement of functional independence. Most of what they were quite effective for the patient. The researchers, conclude that in rehabilitation, as well as in any field, that we do not need the availability of the state-of-the-art equipment which requires money but instead, we need to exhaust all available resources and materials to achieve our goal. The respondents, in view of the patient's problems upon evaluation, have recommended several home modifications of patient's tools and environment. Modifications such as putting a firm comfortable mattress on the bed for the patient's ease and comfort. Putting ropes around and inside patient's house so patient can move about on her own, alteration of her wheelchair and more importantly, instructions to patient and family on the importance of such modifications and additional suggestions in the aid of preventing further complications.
