Acquired perforating dermatosis in a patient with chronic kidney disease : a case report

Abstract
This is a case of a 50-year old female. known case of Type 2 Diabetes Mellitus and Chronic Kidney Disease Stage V maintained on regular hemodialysis three times a week since 2015. Three months prior to consult, she presented with severe pruritis intractable to oral antihistamines or topical antipruritic agents. Chronic scratching and rubbing of the skin eventually led to the development of generalized papules and nodules with a central keratotic plug, starting form the nape, upper extremities, trunk, then lower extremities. Patient consulted with a dermatologist and underwent punch biopsy of the said lesions which scale crusts with numerous neutrophils, lymphocytes, fibrin, altered collagen, and elastic fibers. Patient was given topical epidermal growth factor (EGF) cream and Clobetasol cream which showed significant improvement and regression of the skin lesions.
