A case of left isolated internal iliac artery aneurysm (IIIAA) sacular type with left iliopsoas abscess

Abstract
Isolated Internal Iliac Artery Aneurysm (IIIAA) is a rare case with high mortality due to high incidence of rupture. The location of aneurysm deep in the pelvis makes it difficult to detect by physical examination. The clinical presentation may remain asymtomatic until it ruptures or it makes a local compression to adjacent pelvic structures. Psoas abscess is infrequently in daily clinical practice and has not been reported to be associated with Isolated Internal Iliac Artery Aneurysm (IIIAA). The clinical symptom of psoas abscess is also highly variable making the early diagnosis difficult. This is a case of a 61 year old Filipino male who presented with persistent low back pain and intermitent fever. On physical examination there was left CVA tenderness , erythematous, swollen and tender left leg and faint left peripheral pulses. Whole abdominal CT scan revealed an iliac artery aneurysm and psoas abscess. Adequate antibiotic was given and ultrasound guided percutaneous drainage of ilopsoas abscess was done. Aorto-iliac angiogram was also done and showed giant lobulated saccular aneurysm of the left internal iliac artery with indicative thrombus formation.
