Ramsay-hunt syndrome presenting in a 28 year old Filipino male in immunocompromised state

Abstract

We report a case about a 28 year old male diagnosed with Ramsay Hunt Syndrome presenting with an eight day history of throbbing pain on the right ear primarily involving the external auditory meatus with noted paresthesia prior to eruption of vesicles, multiple vesicular to bullous lesions located on the right side of the face involving the right ear, cervical area and chest area at C2 to C5, T1 and T2 dermatomal level associated with ipsilateral facial numbness. No adjunctive diagnostic procedures and radiologic imaging were done. The patient was treated with Acyclovir IV and noted resolution of symptoms during the 7 days of hospital stay. Herpes zoster oticus occurs in 5/100,000 population and in 0.2% of patients presenting with Herpes zoster. This condition is the 2nd most common cause of peripheral facial palsy among adults and careful evaluation should be done at all times.
