Survey of coronary heart disease patients that meet prevention target recommendation based on 2012 European guidelines on cardiovascular disease prevention in De La Salle University Medical Center

Abstract

This is a cross-sectional survey study conducted at De La Salle University Medical Center hospital in Philippines. Consecutive patient aged 18 years or over with established coronary heart disease more than 6 month consult in outpatient department or admitted at the ward were included. A total of 209 consecutive patients were included in this study. The mean age is 62.34 (SD±12.77) year. The distribution of gender is almost equal. Majority of diagnosis criteria is ACS (53.6%). 39.7% patient have history of smoking and only 8.2% still a persistent smoker at interview. The most common comorbidities were dyslipidemia (88.5%), hypertension (74.6%) and diabetes mellitus (40.2%). One third patient have family history of premature cardiovascular disease. Three highest prevalence of risk factors were low HDL cholesterol (HDL < 1mmol/L in men, or < 1.2mmol/L in women) (77.5%), central obesity (waist circumference ≥90cm in men or ≥80cm in women) (71.7%) and less physical activity (68.9%). The prevalence of patient with BMI 23-24.9) kg/m2 BMI 25-29.9 kg/m2 were 23.9% and 28.2% respectively. One of out ten patients have obesity (BMI>30kg/m2). One out of seven patient have blood pressure < 140/90. Only 41.6% patient with DM at target Hba1c < 7%. Only 31.1% patient report to have at least 30 minutes, 3x/week, moderate to vigorous activity. LDL cholesterol < 1.8mmol/L only seen in 14.8% patient. Smoking cessation noted in 79.5% smoker. The most frequent medication prescribed to patients were antiplatelet (91.4%), followed by statin (80.4%) and nitrate (57.9%). Almost all of smokers (90.3%) reported had received advice to stop smoking, but only 34% of coronary patients reported that they had been advised regarding recommended physical activity target by physician. There are still high prevalence of risk factors, include modifiable risk factors in coronary heart disease patient. The prevalence of patient meet the lifestyle and risk factors target recommendation also still far from optimal.
