Anatomical variations of the cystic duct and artery in relation with the incidences of intraoperative events during laparoscopic cholecystectomy

Abstract

A video review of all laparoscopic cholecystectomies done from January 2014 to December 2016 was done. Exclusion criteria where those diagnosed to have acute cholecystitis preoperatively and intraoperatively. Procedures that did not achieve the critical view of safety was also excluded due to difficulty in correctly identifying the anatomy. All laparoscopic cholecystectomies were assisted by an expert laparoscopic surgeon (done more than 100 cases) as per department policy. Cystic duct and artery anatomy were noted, with or without anatomical variations. The corresponding anatomy was compared to the incidences of intraoperative events and, if any, intraoperative complications. Observers were the researchers together with a senior resident how were trained by an expect laparoscopic surgeon . The study concluded that variations in the cystic duct anatomy, most noted in the short cystic duct, increases the probability of intraoperative events.
