Abstract
Two hundred and forty eight patients with endometrial pathology were included in this retrospective review, case control diagnostics study. Ultrasound reports of the population with IETA features were reviewed retrospectively and presence or absence of endometrial carcinoma based on histopathologic report were determined. Endometrial thickness 89.6% (95% CI 83.8% to 93.8%) and the presence of vessels 76.7% (95% CI 69.4% to 82.9%) were the parameters in IETA that were sensitive in the diagnosis of endometrial carcinoma. The following features were relatively specific in detecting endometrial carcinoma: presence of adenomyosis (92.9%, 95% CI 85.3% to 97.4%), irregular EM junction (98.8%, 95% CI 93.6% to 100%), irregularly branching vessels. Higher positive predictive values were seen for irregular EM junction 95.8% (95% CI 78.9% to 99.9%) and irregularly branching vessels. All features had low negative predictive values. In terms of accuracy, all features had equivocal diagnostic accuracy to detect endometrial carcinoma with 95% confidence intervals crossing or below 50% accuracy.
