The first questionnaire issued and applicable to the Administrator of the disability-related agency/institution. The second type of questionnaire given to the supervisor of DSWD and the Head Officer of DOH. The data from the questionnaire analyzed. Based on the findings, the researchers concluded (1) the results showed Dasmariñas was the town singled out to have disability-related institutions rendering predominantly physical therapy and psychiatric consultation, while in Imus and Silang there were no such agencies existed. (2) The study aimed to upgrade the referral system of the disabled individuals in Cavite by determining the number, location, and services rendered by these agencies.
