Abstract
This study aimed to determined the common errors that PT interns of batch 1999 committed in making their endorsement notes during their April-September rotation in Jose P. Rizal Memorial Foundation, Inc. (JPRMFI) and University Medical Center (UMC). This descriptive study utilized an endorsement checklist made by the researchers based on the format of Endorsement/Outgoing notes in Dr. Ramona Luisa Pablo-Santos "Documentation and History Taking." Fifty (50) endorsement notes made by 17 interns from JPRMFI and 33 interns from UMC were studied. Data were then tallied according to (1) completeness, and/or the presence and/or absence of the given criteria on the endorsement checklist and (2) structure/format.
