Abstract
This study was based on descriptive non-experimental type of research. The respondents of the study were 25 BSN IV-2 students and 25 staff nurses at JPRNMRC. Self-administered, checklist type of questionnaire was used to collect data from the respondents. The statistical treatment used in the study was arithmetic mean and t-test. Based on the stud made, the hypothesis revealed that there was no significant difference on the perception of BSN 4-2 students and staff nurses on the extent of utilization of kardex at KPRNMRC was rejected. Because the data showed that there was significant difference on the perception of BSN 4-2 students and staff nurses, the BNS 4-2 students perceived that the kardex in JPRNMRC were no properly utilized at all, while the staff nurses perceived that it is utilized at JPRNMRC. The data collected proved that even the staff nurses in JPRNMRC were not always utilizing the kardex as a means of source of centralized pertinent information about the patient and also as an important tool of communication between the other health team. Even though they know the importance of kardex and the usually have another sheet of paper for endorsing. Understaffing may also be a factor.
