Diabetic ketoacidosis as a rare manifestation of latent autoimmune diabetes of the adult

Abstract

Latent autoimmune diabetes of the adult (LADA) is a subtype of type 1 DM affecting 5-10% of patients with phenotypic type 2 diabetes which shares genetic features of both types. Patients are non-insulin requiring for at least 6 months after diagnosis; thus, they are commonly misdiagnosed as type 2 DM. Beta cell failure is slowly progressive, hence acute hyperglycemic complications are rare as clinical manifestations. Here we present a case of a 39-year old Filipino female initially managed as a case of type 2 diabetes for the past 3 years with oral hypoglycemic agents (OHAs), who presented with pneumonia and diabetic ketoacidosis, with admission HbA1c of 6.7%. She eventually tested positive for GAD 65 antibodies three months after discharge and was diagnosed with LADA.
