Abstract

Herlyn-Werner-Wunderlich (HWW) syndrome is rare congenital anomaly consisting of uterovaginal duplication with obstructed hemivagina and ipsilateral renal agenesis. A case of an 11 year old child presented, with pain in the right lower quadrant area and dysmenorrhea, present since menarche and worsening for the past few months. Transrectal ultrasound revealed a right adnexal mass and vaginal vault mass representing blood. A right adnexal mass and renal agenesis of the right kidney with compensatory hypertrophy of the left kidney was detected by whole abdominal CT scan. Upon laparoscopy, hematocolpos, transverse vaginal septum and right endometriotic cyst were noted. Lapasroscopic right ovarian cystectomy with adhesiolysis and excision of transverse vaginal septum were done. On the 4th postoperative day, patient went home without any specific complications. Prompt and accurate diagnosis and treatment of this syndrome can significantly improve the life of this patient and prevent future complications.
