Odd occlusion : cutaneous thrombosis in paroxysmal nocturnal hemoglobinuria a case report

Abstract

A case of a 51 year old Filipino male with episodes of intermittent tea-colored urine diagnosed and managed as a case of PNH and presented with widespread hemorrhagic bullae and eschars. Skin biopsy showed cutaneous thrombosis. Diagnosis of PNH was based on: (1) clinical presentation of tea-colored urine; (2) positive fluorescently labeled inactive toxin aerolysin (FLAER), cytopenia (anemia and thrombocytopenia). Diagnosis of cutaneous thrombosis was based on: (1) generalized petechiae and purpuric rashes that ulcerate and necrosed (2) histopathologic findings of hemorrhage in the dermis with capillary thrombosis consists of fibrin, RBC, RBC fragments and platelets compatible with thrombotic purpura.
