Recurrent left atrial myxoma after excision of a triple chamber myxoma in a 20 year old male : a case report

Abstract
This is a case of a 20 year old male who was diagnosed with triple chamber myxoma (RA, RV, LA) in January 2015 and underwent excision of tumors without immediate postoperative complications. On his third year of follow up, he developed native aortic valve infective endocarditis and was incidentally diagnosed with recurrent left atrial myxoma. When infection was resolved, he underwent repeat surgery for aortic valve replacement and excision of recurrent atrial myxoma. Post operatively, he had multiple episodes of arrythmia and developed cardioembolic infarct and seizures, sepsis, acute kidney injury, and acute heart failure, subsequently expiring from malignant arrhythmia. Recurrences of cardiac myxomas can happen within a few months to as late as 20 years after excision. Incomplete resection of the tumor is the most common cause of recurrence and may be the reason for this case of sporadic triple chamber cardiac myxoma. This study highlights the incidence of recurrences, diagnosis, symptoms and complications of recurrent cardiac myxomas. This is the first local reported case of recurrent cardiac myxoma from a triple chamber or any chamber myxoma and also the first local reported case of cardiac myxoma developing acute heart failure occurring simultaneously with infective endocarditis.
