Abstract
The goal of health screening is to prevent disease or prevent worsening. A health screening strategy involves not just the use of a screening test, but also the confirmatory test and any therapy that may be needed if early disease is detected. Trials on screening may be direct or indirect. Compared to direct trials, the main disadvantage of indirect trials is that they fail to assess the screening strategy in its entirety. Validity criteria for appraisal are the same as those used in other clinical trials in general. The absolute reductions in screening trials are typically small, because subjects are generally healthy. Thus, it is difficult for patients to experience benefit, yet they are rendered susceptible to harm. For this reason, it is important to assess the harms arising from all components of the screening strategy, including the screening test, the confirmatory test, and any therapy that may follow.
