Abstract

In rehabilitation medicine, PT entails a lot of responsibilities and skills. One of these skills is with regards to documentation and charting , which serves as method of communication among other health personnel and considers a baseline documents. It is vital in monitoring the conditions the patients, establishing treatment programs also as a source of data for research. The main objective of the authors is to determine the completeness done by PT Interns of Batch 2001 in making Initial Evaluation for Chronic Low Back Pain during their rotations at Jose P. Rizal Memorial Foundation Inc. and De La Salle University Medical Center. The researchers gathered 50 initial evaluations , 20 from jose P .Rizal Memorial Foundation Inc. and 30 from De La Salle University Medical Center made by regular PT Interns Batch 2001 from April up to February 2000 by searching names of out patient who was diagnosed to have chronic low back pain. The researchers in recording the frequency of errors made checklist and tally sheet. Complete entries were only observed under Family History, Vital Signs, Manual Mucles Testing and Special Tests. The 3 specific areas in initial evaluation that have the most common errors were Pat Medical History, Abdomen and Problem List. In relation with the study made by Puelong et al. entitled "Common Errors Committed by De La Salle Health and Science Campus PT Interns Batch 1998 in 3 selected Satellite Clinics of the Institution in making their Initial Evaluations, 1999" which revealed that the entries under Chest and Lungs, Back and spine, Postural Analysis, Gait Analysis & Treatment goals had the most incomplete entries under the 3 general classifications of clinic conditions.
