Abstract
To gather a more in-depth information, the study accepted an explanatory mixed method research design, which gathers data through both quantitative and qualitative methods. The quantitative method of the study was used to determine the current PT practices in terms of each of the seven pillar of DCC with the use of a questionnaire to gather data. An open-ended interview was used to learn more about the strength, weaknesses, opportunities, and threats (SWOT) of DCC in the current PT practice. The participants of the study were the perceptions of the CRS satellite centers of DLSMHSI and were gathered through a purposive sampling method. Data gathered for both methods were analyzed through frequency-percentage distribution, weighted mean, and thematic analysis. The results of the data gathered and analyzed showed that DCC was moderately applied into the PT care practice. This also indicated that there was a sustainable framework in the practice despite its inconsistencies. It was further concluded by the results of the SWOT analysis of the DCC being applied into the PT practice. The strength were the following: (1) competent PT care through the removal of barriers, (2) affirmed continuation of care, and (3) effective collaborative efforts in the health care team. In contrast, the weaknesses displayed the inconsistencies of the practice which were- internal problems faced by the practice as to the lack of resources, adequate training, and proper DCC integration. This was also supported by the external factors that may affect the PT practice in terms of opportunities. This involved collaborative efforts with stakeholders, affiliation of academic institutions, and threats-that involves the new trends and updates and partnerships with the local government unit (LGU).
