Abstract
Caring for patients at the end of life is among the most emotionally and ethically demanding responsibilities of intensive care unit (ICU) nurses. Beyond medical interventions, they provide comfort and support to families and navigate complex decisions within high-pressure environments. Given these challenges, this study explored the roles and challenges ICU nurses face in delivering end-of-life care (EOLC) and examined how age, gender, hospital level classification, and length of work experience influence their practice. This study is anchored in Lydia Hall’s Care, Cure, Core model and Ida Jean Orlando’s Nursing Process. A descriptive quantitative research design was utilized using a structured questionnaire administered to ICU nurses in Level 2 and Level 3 hospitals in Cavite. Total enumeration sampling was applied, targeting all ICU nurses who met the inclusion criteria. Data were analyzed using an independent t-test, ANOVA, and post-hoc analysis. ICU nurses excel in symptom management and holistic patient care, but face significant challenges, including time constraints, limited resources, and emotional strain. Age and gender showed no significant impact, but hospital level classification influenced the “Care” aspect of EOLC, and length of work experience played a key role. Nurses with two to three years of experience reported the highest complexity in their roles, reflecting transitioning challenges. Despite these challenges, ICU nurses remain dedicated to providing compassionate care. Hospital policies may focus on ensuring resource equity, mentorship, and strengthening coping strategies. Whilst future research should explore the long-term effects of emotional distress of EOLC on nurses.
